Welcome to the Mainline Health Wilmot Medical Clinic, conveniently located in central Wilmot on McCombs Street. Our hours of operation are Monday - Thursday 8:00 AM - 4:30 PM and 8:00 AM - 12:00 PM on Friday. 

Wilmot is a small community, and we are the ONLY healthcare facility in the area. We are here for all your healthcare needs. We offer Primary Care, including anything from routine/sick visits, routine bloodwork, check-ups, to wellness exams, and more. 

We understand that there may be limited resources in the area, but we are here to help. Did you know we have a Community Health Worker Program? The purpose of the CHW Program is to provide resources to our patients and communities. If you have a need for transportation, employment and workforce, or housing, this program would be a great option. Our team includes nurses, care coordinators, dentists, and other healthcare providers to assist with the services you need. Once you join the program, you will be assigned a primary Community Health Worker to facilitate your needs. This person will use the tools available to them to find the necessary community resources needed to help you.

We also offer a Care Management Program. The Care Management team is available 24/7 to assist you with scheduling appointments, transportation needs, paying for your prescriptions, disease education and management, an so much MORE.
